Before commencing medical cannabis treatment, we will need to gather some essential information from you.

Your eligibility for medical cannabis treatment is assessed online

The first question is, "What primary condition are you seeking treatment for?". We can request a copy of your medical records from your GP on your behalf, or you can grab them yourself and upload them onto our secure site. If you have seen a specialist about your condition, sharing any consultation notes and reports would also be helpful, as these can provide valuable insight into your medical history.

If you do not yet have access to your full medical records, Releaf also offers a self-certification option once your account has been created and the initial evaluation form has been completed. This allows you to declare previously tried treatments that may not appear on your NHS records and upload supporting information directly through your account for our world-class clinical team to review as part of triage.

Medical cannabis assessments include a detailed health questionnaire
